Sprouts Farm and Forest Kindergarten
2023-2024 Application

Thank you for your interest in Sprouts Farm and Forest Kindergarten. Children turning 3 by June 15, 2023 are eligible for enrollment in 2023-2024. Please fill out the application below. Once we receive your application we will contact you to schedule a visit. Visits take place in the Spring during our school day so that you can see and experience first-hand what it is like to be a Sprout and determine if it is a good fit for your family and child. Enrollment form and fee can then be submitted to hold a spot for the upcoming school year.
